The pharmacovigilance guidelines are in the process of being updated, and companies will be informed directly by Medsafe when these are finalised.

Please email CIOMS forms to carmreport@health.govt.nz. Forms already sent to the previous email address have been transferred by CARM and received by Medsafe. 

There is no need to resubmit CIOMS forms. 

Alternatively the webform can be used If your organisation does not use a CIOMS form. If there is not a relevant field, include information in the additional comments field at the end of the webform.
